
Sports Physicals

All participants are required to have a current physical on file, within last three years. If your physical is more than three years old, you will not be allowed to register or participate.

About Sports Physicals
Sports Qualifying Physical Examination forms must be completed by your medical provider. Other medical charts and documents are not valid for sports participation physicals. Completed physical forms need to be turned in to the athletic director at the student-athlete's school.

Where to Get a Sports Physical
- Your family health clinic (typically covered by insurance when combined with annual wellness visit)
- CVS MinuteClinic Sports Physical (usually under $100. Paid out of pocket)
- Register for a free sports physical at Twin Cities Orthopedics (Registration required. Dates vary.)
