Life and Long-term Disability Insurance

Life Insurance

Group Term Life Insurance provides your beneficiaries with lump sum payment in the amount of your life benefit in the event of your death.  The eligibility and amount of the life insurance benefit varies based on your work group as detailed in the collective bargaining agreement or benefit provisions.  Premiums are fully paid by District 196. 

Supplemental Life Insurance – optional additional life insurance coverage available to employees eligible for group term life insurance in the teacher, clerical, food service, custodial, transportation and vehicle technician groups.  Employees may purchase an additional $20,000, $40,000, $60,000 or $80,000 of supplemental term life insurance coverage at their own expense.  Rates are based on age bands and amount of coverage per the life insurance policy with the carrier.  If not elected when initially eligible, application for coverage is subject to carrier underwriting approval.  

Benefits will be determined and paid as described in the life insurance policy with the carrier, Madison National Life.
 

Long-Term Disability Insurance

Long-Term Disability (LTD) provides income replacement benefits after an injury or illness that results in your continuous disability causing you to be unable to work for more than 90 calendar days (known as the elimination period).  The LTD benefit is the greater of 66 2/3% of your monthly Covered Earnings, reduced by any Other Income Benefits, up to the maximum LTD benefit for your work group, or the LTD minimum benefit.

Eligibility for LTD insurance is based on the collective bargaining agreement or benefit provisions for your position and if eligible enrollment is mandatory. Employees pay the full premium for this coverage by payroll deduction and as a result LTD benefits are not subject to state and federal tax.
