1. Students
1.1 Parents or guardians shall maintain up-to-date health and emergency contact information.
1.2 In case of the serious injury or illness of a student in school, the following steps will be taken immediately:
1.2.1 The school nurse will be called;
1.2.2 Emergency telephone number 911 will be called, if deemed necessary, and
1.2.3 The student's parent, guardian or other person designated as an emergency contact will be called. If none of them can be reached, the school staff will make whatever arrangements are necessary for the health of the studen
1.2.4 In case of an emergency involving a student, the school nurse or other school staff trained in the use of the automatic external defibrillator and cardio-pulmonary resuscitation (CPR) may use the defibrillator on the student, as
2. Medical Incident Report for Students, Community Education Participants and Visitors
2.1 In case of the serious injury or illness of a student, a Medical Incident Report for Students, Community Education Participants and Visitors (District Procedure 506.2.1.4P) will be completed and kept in the student’s cumulative file.
2.2 In case of the serious injury or illness of a community education participant, a Medical Incident Report for Students, Community Education Participants and Visitors (District Procedure 506.2.1.4P) will be completed and kept on file by the appropriate Community Education office.
2.3 In case of the serious injury or illness of a visitor, a Medical Incident Report for Students, Community Education Participants and Visitors (District Procedure 506.2.1.4P) will be completed and kept on file in the school or building where the incident occurred.
3. Student Seizures
3.1 Seizure action plans
3.1.1 School personnel will collaborate with the parent or guardian to implement a seizure action plan when an enrolled student’s parent or guardian has notified the school that the student has a diagnosed seizure disorder and has seizure rescue medication or medication prescribed by the student’s licensed health care provider to treat seizure disorder symptoms approved by the United States Food and Drug Administration.
3.1.2 A “seizure action plan" means a written individualized health plan designed to acknowledge and prepare for the health care needs of a student with a seizure disorder diagnosed by the student's treating licensed health care provider. A seizure action plan must:
3.1.2.1 Identify a school nurse or a designated individual at each school site who is on duty during the regular school day and can administer or assist with the administration of seizure rescue medication or medication prescribed to treat seizure disorder symptoms approved by the United States Food and Drug Administration; and
3.1.2.3 Be provided to the person identified under section 3.1.2.1; and
3.1.2.4 Be provided to the employee or volunteer responsible for the supervision or care of a student with a diagnosed seizure disorder, along with the name or position of the employee identified under section 3.1.2.1 and the method by which that person may be contacted in an emergency; and
3.1.2.5 Be filed in the office of the school principal or licensed school nurse or, in the absence of a licensed school nurse, a professional nurse or designated individual; and
3.1.2.2 Require the employee identified under section 3.1.2.1 to receive specific training that includes seizure medications, recognition of signs and symptoms of seizures and appropriate steps to respond to seizures.
3.2 General training. The school district will provide all licensed school nurses or, in the absence of a licensed school nurse, a professional nurse or designated individual, and other school staff working with students with self-study materials on seizure disorder signs, symptoms, medications, and appropriate responses.
