COLLABORATIVE - WHAT IS IT?

As of January 1, 2006, the ISD 196 Family Services Collaborative (FSC) has merged with the Children's Mental Health Collaborative forming the new District 196 Collaborative Hub. This continues to be collaborative partnership among parents, community agencies and schools. This partnership was formed to enhance the stability and health of children and families. It supports the delivery of family support services tailored to individual needs and is focused on prevention and early intervention. The goal of the collaborative is the prevention of out of home placement of children. Through the clusters and the Hub, collaborative partners include representatives from:

HOW IS THE GOAL ACCOMPLISHED?

It is accomplished by servicing families more holistically and developing and implementing family driven action plans. Plans are built around family goals and strengths. Staff and families work together using the Individual Family Resource Plan to specify family goals, strategies, responsibilities, and funding sources.

WHO IS ELIGIBLE TO BE SERVED THROUGH THE DISTRICT 196 COLLABORATIVE?

Children, birth to 18, and their families residing in the ISD 196 attendance area, who would benefit from a holistic collaborative approach to meeting multiple needs

WHAT RESOURCES ARE AVAILABLE TO FAMILIES THROUGH THE COLLABORATIVE?

  • Families - Families are their own best resource.
  • The family’s community - The community where the family lives is an invaluable resource available to families.
  • The family’s support team - Family members may look to resources outside the support of family and friends, to more formal resources such as school, human services, and public health.
  • Information and Referral - Information is power to those who use it. Collaborative members help families identify and access resources and services in the community.
  • Family Driven Resource Plans - The plan is designed to help families recognize their strengths, set their goals, and take action on their own behalf.
  • The Flex Fund - One tool that can be utilized by family support teams in conjunction with the Family’s Resource Plan. This fund is used when other sources of funding do not exist, a timely response is needed, and the requested purchase builds capacity in families and is for a one time, not an ongoing need.

HOW ARE THESE RESOURCES ACCESSED?

Collaborative resources are accessed through the Collaborative Cluster which represents the communities of Apple Valley, Eagan, and Rosemount. The cluster is composed of representatives from agencies and schools in those areas who serve children and families. This model allows the cluster to customize the collaborative to meet the needs of their community.  At this time, the clusters meet in every other month to access the needs of the community and share resources.
