Sports Physicals

Any student in grades 7-12 participating in interscholastic athletics, danceline, or cheerleading must have a current physical examination on file. This physical must be done within the past three years and may be performed by a physician, osteopathic physician, chiropracter, or nurse practitioner working under the supervision of an MD.
If you have a child who intends to participate in these activities in the coming school year and does not have a current physical on file at their school, contact their health care provider to schedule a physical examination.
After major surgery or serious illness or injury, a physician must certify in writing the student's readiness for participation.
The completed sports physical exam form should be submitted to your child's school. In order to meet the requirement, the form must include the following:
- date the physical was performed
- signature of the doctor who conducted the exam
- statement that the child is cleared to participate in the sports and/or activities they intend to pursue.
Completed physical examination forms are kept in the school athletic office or nurse's office depending on the individual school.
In addition to having a health physical, a health questionnaire provided by the Minnesota State High School League must also be completed annually and signed by the parent/guardian.
